About INTERACT II

The INTERACT II Program is designed to improve the quality of nursing home care by providing tools and resources to staff that will help to Reduce Avoidable Acute Care Transfers. The program is built around a number of practical tools designed to:

  • Aid in the early identification of a resident change of status
  • Guide staff through a comprehensive resident assessment when a change has been identified
  • Improve documentation around resident change in condition
  • Enhance communication with other health care providers about a resident change of status

The first version of the INTERACT program was developed by the Georgia Medical Care Foundation, the Medicare Quality Improvement Organization for the state of Georgia, with the support of a contract from the Center for Medicare and Medicaid Services. The development of INTERACT II has been supported through a generous grant from The Commonwealth Fund.

How do I use this website?

This website is designed to enable a designated facility leader (Champion) and his/her team to implement the INTERACT II tools and processes to Reduce Avoidable Acute Care Transfers. If this is your first time visiting this website, we recommend that you follow the prompts below to orient yourself to the program and the toolkit. If you are a return visitor, you can easily go to the tools/areas that you need. We anticipate that you will make frequent visits as your facility begins to use the toolkit.

What is a Champion and WHY DO I NEED ONE?

An Interact II Champion is the nursing home’s coach for implementing the INTERACT II program.

Selection of a Champion for this program is essential to implement the new tools and processes, to keep the ball rolling, to insure that the program stays visible, and to sustain the focus on reducing Acute Care Transfers.

Credits

The INTERACT II Tools, educational materials, and implementation strategies were developed by Drs. Joseph Ouslander, Gerri Lamb, Alice Bonner, Ruth Tappen, and Ms. Laurie Herndon with input from a variety of direct care providers and national experts in a project supported by the Commonwealth Fund based at Florida Atlantic University. Initial version of the INTERACT Tools was developed by Dr. Joseph Ouslander and Mary Perloe, MS, GNP at the Georgia Medical Care Foundation with the support of a special study contract from the Center for Medicare and Medicaid Services. This website was created by the Stein Gerontological Institute.
